- Supporting the learner and their family and carers in developing attitudes, beliefs, knowledge and skills to self-manage diabetes
- A commitment to the DESMOND philosophy and principles
Entrants need to demonstrate the following criteria:

DESMOND teams should nominate individual patients who have gone on to change their lives as a result of the course. This could look like:
